Life claims information

 

In the event of a claim please contact the Life Insurance Department at Citadel Insurance on 2557 9000 or send an email to ci-life@citadelplc.com.

We will be happy to guide you through the process however, payment of any benefit as outlined in the terms and conditions of your policy schedule will be subject to us receiving notification in writing within 3 months of the happening of the event giving rise to a claim and subject to the Company receiving at its Head Office the following details;

  1. the original Policy Document
  2. proof that the event upon which the benefit (s) is/are payable has occurred
  3. proof of the date of birth of the Life insured
  4. proof of identity, and the right to receive the Benefit (s) of the person making the claim
  5. original bank pledges and discharge forms where applicable.
